Certaines sont plac\u00e9es sous la r\u00e9tine (sous-r\u00e9tiniennes), d'autres reposent sur la r\u00e9tine (\u00e9pi-r\u00e9tiniennes), et certaines contournent compl\u00e8tement l'\u009cil en stimulant directement le cerveau par des implants corticaux. Chaque approche pr\u00e9sente des avantages particuliers selon la maladie trait\u00e9e.<\/p>\n\n\n\n<p>La pr\u00e9sentation du Dr Palanker s'est concentr\u00e9e sur la proth\u00e8se r\u00e9tinienne PRIMA, d\u00e9velopp\u00e9e par Pixium Vision et actuellement en essais cliniques. Ce syst\u00e8me est con\u00e7u pour les personnes atteintes d'atrophie g\u00e9ographique, une forme avanc\u00e9e de d\u00e9g\u00e9n\u00e9rescence maculaire li\u00e9e \u00e0 l'\u00e2ge (DMLA) s\u00e8che.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Comment fonctionne PRIMA<\/strong><\/h3>\n\n\n\n<p>PRIMA est une minuscule puce \u00e9lectronique plac\u00e9e sous la r\u00e9tine, \u00e0 l'endroit o\u00f9 les photor\u00e9cepteurs ont disparu. Les patients portent des lunettes sp\u00e9ciales \u00e9quip\u00e9es d'une cam\u00e9ra qui capte les images et les projette sur l'implant gr\u00e2ce \u00e0 une lumi\u00e8re infrarouge invisible. L'implant stimule ensuite les cellules r\u00e9tiniennes restantes, permettant au cerveau de percevoir des formes, des lettres et des motifs.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Ce que vivent les patients<\/strong><\/h3>\n\n\n\n<p>Dans des essais cliniques men\u00e9s en Europe aupr\u00e8s de plus de 40 participants, la majorit\u00e9 des patients ont gagn\u00e9 en moyenne cinq lignes sur une \u00e9chelle d'acuit\u00e9 visuelle. Certains ont pu lire de gros caract\u00e8res, reconna\u00eetre des objets et reprendre des activit\u00e9s quotidiennes comme cuisiner ou jouer aux cartes. Pour l'instant, la vision restaur\u00e9e se limite \u00e0 des nuances de gris plut\u00f4t qu'\u00e0 la couleur, mais de nombreux participants jugent cette am\u00e9lioration significative et utile. Avec l'entra\u00eenement, le cerveau apprend \u00e0 mieux interpr\u00e9ter ces nouveaux signaux visuels.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Limites et perspectives<\/strong><\/h3>\n\n\n\n<p>Si les r\u00e9sultats pour la DMLA sont encourageants, il est important de souligner que les m\u00eames niveaux de vision ne sont pas encore possibles pour les maladies r\u00e9tiniennes h\u00e9r\u00e9ditaires (MRH) telles que la r\u00e9tinite pigmentaire ou la maladie de Stargardt, qui causent des dommages plus \u00e9tendus et durables \u00e0 la r\u00e9tine. Dans ces cas, les proth\u00e8ses pourraient offrir seulement la perception de la lumi\u00e8re ou des contours. Des recherches sont en cours pour \u00e9valuer le potentiel de PRIMA aupr\u00e8s de ces patients.<\/p>\n\n\n\n<p>La prochaine g\u00e9n\u00e9ration d'implants est con\u00e7ue avec des pixels plus petits, permettant une image plus nette et la lecture de caract\u00e8res plus fins. Des innovations logicielles, telles que le zoom \u00e9lectronique et la stabilisation d'image, sont \u00e9galement en d\u00e9veloppement.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Un avenir prometteur<\/strong><\/h3>\n\n\n\n<p>Le syst\u00e8me PRIMA demeure en phase d'essais cliniques et n'est pas encore disponible comme traitement standard. Les processus d'approbation peuvent prendre plusieurs ann\u00e9es, mais les avanc\u00e9es se poursuivent.<\/p>\n\n\n\n<p>Le Dr Palanker a rappel\u00e9 que, tout comme les implants cochl\u00e9aires ont transform\u00e9 la prise en charge de la perte auditive, les proth\u00e8ses r\u00e9tiniennes pourraient un jour devenir une option courante et d\u00e9terminante pour traiter la perte de vision.
